Prior to your initial visit to Kruse Park, please take a moment to download and complete our New Patient Paperwork.

Having this paperwork filled out in advance will save you time and allow us to serve you better. Please make sure you also bring a copy of your insurance card to your appointment.

WHAT TO EXPECT:
On your first visit, you will have a complete examination and consultation with one of our chiropractic physicians. The exam will include ranges of motion, muscle assessment, and neurological and orthopedic testing. We may refer you for imaging, if necessary.

After the exam, we will discuss what we’ve found and recommend a course of treatment. If you’re a candidate for chiropractic care, we will perform your first adjustment on this visit, unless red flags are present. We never perform treatment without first explaining what we’re going to do. If you’re not a candidate for chiropractic care, we can recommend some of the top doctors in our area who specialize in the care you need.

We accept most major insurances, as well as worker’s compensation and automobile accident claims.
